Appointment Policies and Procedures
- Schedule before or during the class(es) you need excused* - delaying scheduling may result in not being excused from classes
- **If you are on campus, we do not excuse classes without an appointment**
- *We understand that appointments fill up quickly. If there are no available appointments before the class(es) you need excused, still book the first available for that day.
- If there are no appointments for the rest of the day, please email the clinic with your symptoms and your cell phone number. We will contact you when available.
- No walk-in appointments unless it is an emergency (ex: anaphylactic shock)
- If you need medical attention after hours, please contact your CC or go to the Bryan Desk.
- We cannot excuse classes after hours if you did not

Medication Policy and Instructions
Prescription medications policyAll residential students are required to update Blackbaud regarding all prescription and over the counter medications. Students MAY NOT keep the following categories of medications in their room:
- Antidepressants (ex: Zoloft, Prozac, Lexapro, Wellbutrin and others)
- Antipsychotics (ex: Risperdal, Seroquel, Abilify and others)
- Anxyolitics (ex: Ativan, Xanax, Klonopin and others)
- Mood stabilizers (ex: Lamictal, Depakote, Tegretol and others)
- Stimulants (ex: Ritalin, Adderall, Concerta, Vyvanse and others)
Over the counter medications Students may keep over the counter medication(s) and some prescription medication(s) in their room.
Prescription medications pick up/drop off:
- Students taking psychotropic medications must fill out Form F (blackbaud) and sign the consent on page 2.
- The student should bring the medications to the clinic for weekly distribution at the beginning of the new semester or when a new prescription is added.
- Students must come to the clinic weekly to pick up a weeks worth of medications.
- Students may keep only one week supply of the prescribed medications in their room at any time.
Important tips regarding medications:
- Students are responsible for their own course of treatment by taking their medication(s) as directed and by keeping only one week's worth of medication in their room at a time.
- It is not the responsibility of the clinic to remind students to pick up their medication(s). Parents should check in with their student regularly to ensure that they are taking their medication(s) as directed.
- Misuse, abuse, selling or giving prescribed medications to others is strictly prohibited and is a violation of the NCSSM Code of Conduct which may result in disciplinary action up to and including dismissal from NCSSM.
- NCSSM assumes no responsibility for the use or misuse of such medication.

The Durham NCSSM clinic partners with Gurley's Pharmacy to fill and deliver 4 weekly blister packs to the clinic for distribution.

Medical Forms
All families and students at NCSSM, not just the Residential program needs to upload the Junior Medical Packet, Senior Medical Packet and any Individualized Health Plans to "Blackbaud". The medical tab is only available to the parent profiles in Blackbaud, so parent profiles will need to be the ones that submit medical packets and allergies to the clinic.
Blank versions of both the Junior Medical Packet and Senior Medical Packet are linked her.
The Junior Medical Packet should be uploaded by the due date (typically mid May) even if the last physical date is over a year. The forms include the NCSSM physical form and the NCHSAA Physical form (A and B) both signed by a Primary Care Provider, Immunization Records, TB form signed by PCP and all pertinent allergy action plans. If your student has a new physical or updated immunization record after the due date, please update/upload in Blackbaud when available. Students must have all required forms and immunizations in order to move into the residence halls.
For varsity athletics, a physical less than a year old for the entire sports season must be submitted to the NCSSM clinic before tryouts or participation can occur
